Request an appointment for a blood test and urine test (for diabetes/kidney review)

Use this service if you are due to have a diabetes or annual CKD (kidney) review, but have not had a blood test and urine test.

You can use this service if you:

• are registered at the surgery
• have been invited to do so

Any subsequent forms should be submitted 1 week after your blood test and urine test.

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
